Clinical Pharmacist (Pharm D)

- Conduct medication therapy management
- Dispense medications
- Review and interpret prescriptions
- Monitor patient health and progress
- Provide immunizations
- Educate patients on medication use
- Collaborate with healthcare providers
- Ensure compliance with regulatory requirements
- Maintain accurate patient records
- Identify and resolve medication-related problems
- Conduct medication therapy management to ensure safe and effective use of medications
- Dispense medications accurately and efficiently
- Review and interpret prescriptions for appropriate dosage and medication
- Monitor patient health and progress to evaluate the outcomes of medication therapy
- Provide immunizations to patients following established protocols
- Educate patients on medication use and possible side effects
- Collaborate with healthcare providers to develop and adjust patient care plans
- Ensure compliance with regulatory requirements related to pharmacy practice
- Maintain accurate and up-to-date patient records in accordance with privacy regulations
- Identify and resolve medication-related problems through communication with prescribers
- PharmD degree from an accredited pharmacy program
- Valid state pharmacist license
- Strong knowledge of pharmacology and medication therapy management
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team
- Attention to detail and accuracy in dispensing medications
- Familiarity with pharmacy law and regulations
- Proficiency in using pharmacy software for maintaining records
- Willingness to learn and adapt to new healthcare practices
- Clinical experience or internships in a healthcare setting preferred
